Why Medication Support Matters

Missed doses, double doses and confusion over changing prescriptions are among the most common reasons older people become unwell or end up in hospital. Memory changes, poor eyesight, stiff hands or simply a busy pill organiser can all make it harder to manage medicines alone.

Our medication support follows national good practice guidance for managing medicines in home care and sits under our CQC registration, with a written plan, risk assessment and a Medication Administration Record (MAR) for every person we support.

Three Levels of Support, Matched to Your Needs

We assess exactly how much help is needed and agree it in your care plan, so you stay as independent as possible.

Prompting

A reminder at the right time for people who can take their own medicines but may forget.

Assisting

Practical help such as opening packaging, reading labels, pouring liquids or passing a drink, while you stay in control.

Administering

Where you cannot manage your medicines yourself, your trained carer gives them as prescribed and records each dose on your MAR.

Support can move between levels as needs change, and we review it at every care plan review.

How We Keep Medication Safe

Medication assessment: We check what is prescribed, how it is stored and the level of support needed.

MAR records: Every prompt, assist or dose is recorded on a Medication Administration Record.

Trained carers: Carers complete medication training and competency checks before supporting anyone with medicines.

Working with GPs and pharmacies: We raise concerns, flag changes and help keep prescriptions up to date.

Safe storage: Advice on keeping medicines secure and separating out-of-date or discontinued items.

Audits and oversight: Records are checked regularly by our management team, with 24/7 on-call support for carers.

Clear escalation: If a dose is refused, missed or causes concern, we follow a clear process and inform the right people.

Clear About Our Role

Our carers provide medication support as part of social care, not nursing. We do not change doses, advise on which medicines to take or carry out clinical tasks unless a healthcare professional has trained and signed off a carer for that specific task. Any questions about the medicines themselves are always referred back to your GP, pharmacist or nurse.
