End of Life Care at Home in Gateshead and Chester-le-Street
For many people, being at home in their final weeks and days, surrounded by familiar things and the people they love, matters more than anything. We are here to help make that possible, with gentle care, quiet presence and support for the whole family.

Care That Puts Comfort and Dignity First
End of life care supports someone who is approaching the final stage of life to be as comfortable, peaceful and in control as possible. At home, that means help with personal care, comfort and daily needs, while the medical side of care is led by your GP, district nurses and palliative or hospice teams.
Our carers work alongside those professionals, following their guidance and keeping them informed. Every package is delivered under our CQC registration with a care plan, risk assessments and daily records, and our team is supported by 24/7 on-call management.
How Our Carers Help
Personal care: Gentle washing, mouth care, changing and freshening, at a pace that suits the person.
Comfort and positioning: Regular repositioning to protect skin and ease discomfort.
Eating and drinking: Small, comforting sips and tastes when wanted, never pressure.
Medication support: Prompting or administering prescribed medicines and recording each dose, with anything clinical led by nurses.
Night care: Waking nights or sleep-ins so no one has to be alone through the night.
Quiet companionship: Sitting with the person, reading, playing favourite music or simply being there.
Recognising change: Noticing signs that things are changing and alerting the nursing team and family.
Honouring Their Wishes
We take time to understand what matters to the person: who they want close by, the music and comforts they love, their faith or cultural practices, and any wishes they have shared about their care. Where an advance care plan exists, we follow it closely and work with the professionals involved to respect it.

Support for Those Who Love Them
This is an exhausting and emotional time for families. We can take on the practical tasks so you can spend your time simply being together, give you the chance to rest, and be a calm, steady presence when you need one. We keep you informed, answer your questions honestly and are there on the phone day and night.
After a death, our carers can stay with you until the right people arrive and help you through those first difficult hours.
