What Is a Care Assessment?
A care assessment is a relaxed, face to face conversation with one of our experienced team, usually in the home of the person who needs support. It helps us understand daily routines, health and mobility, home safety, personal preferences and what a good day looks like.
It is not a test and there are no wrong answers. It is simply how we make sure any care we offer is safe, personal and genuinely right for you.
What We Will Talk About
Daily life: Routines, what is going well and what has become harder.
Health and wellbeing: Mobility, medication, memory and any conditions that affect care.
Home safety: Stairs, bathroom access, equipment and falls risks.
Personal preferences: Mealtimes, faith, culture, interests and how you like things done.
Family and support: Who is involved now and how we can work alongside them.
Goals: What you would like care to help you achieve.
Who Should Be There?
Ideally, the person who needs care and anyone who helps support them, such as a partner, son, daughter or friend. If the person cannot take part fully, for example because of dementia, we will involve them as much as possible and speak with those who know them best.
Where Assessments Take Place
At home: The best option, so we can see the home and routines first hand.
In hospital or another care setting: Ideal before a hospital discharge.
By video or phone: Useful when family live far away, followed by a home visit before care starts.
Assessments usually take around an hour. We will arrange a time that suits you, including early evenings where possible.
